Old-fashioned doctors spend whatever time it takes to obtain a good medical history and physical examination. The history and physical exam is not something to be fetishized but something to be recognized as a time saver. An exam finding does, on occasion, make you feel more comfortable with your diagnosis.

” If you’re waiting to be discharged from the hospital, what time you’ll be able to leave is anyone’s guess since one hand often doesn’t know what the other hand is doing. Doctors see patients in rushed 15-minute appointments, often without a cursory physical exam. Do we make patients feel welcome?

For each of them, their final year was filled with a frenzy of consultations, calls to come for another evaluation after I was awakened with a new idea, or hospitalizations for observation and diagnostics. I can think of three patients who fill the “doctor knows the patient is dying and can’t tell” category.

In the Fear of Going Out Era spawned by the COVID-19 pandemic, many patients were loath to go to the doctor’s office for medical care, and even less keen on entering a hospital clinic’s doors. These included: A clinician’s lack of ability to conduct a physical exam (75% of older people concerned about this).

After taking his history and doing his physical exam, I recommended HIV testing. Once he was stable enough to exercise his own will, he informed us that he was leaving the hospital. He remained in the hospital for a couple of weeks and left with a management plan that has kept him alive for years.
