Sometimes we are initiating hospice; other times the patient is moving to be with family who will take responsibility for end-of-life care. That said, he would not hear about hospice care, telling us, and himself, that he might still regain enough strength for the oncologist to treat him. All three died without a clear cause.

She was admitted to the hospital with an MI. There were ST elevations across all the precordial leads and signs of heart failure on physical exam. During her first five days in the hospital VB was transferred between ICU and the cardiology ward three times. She had come to the emergency room with five days of chest pain.
